Individuals who have successfully completed the Build Healthy Habits program and are ready to transition to a more independent lifestyle while maintaining access to professional guidance and accountability.
This program is designed for those who want continued support as they sustain healthy behaviors, continue progressing toward their health goals, and confidently manage their wellness independently.

Transition participants from structured coaching to independent self-management.
Reinforce lifelong healthy habits with periodic professional support.
Promote continued improvements in physical function, strength, and overall well-being.
Prevent relapse through ongoing accountability and timely guidance.

What You'll Get
Program: Ongoing monthly membership
Independent Wellness
Follow your personalized exercise and wellness plan independently while applying the knowledge, skills, and habits developed during the previous phases.
Monthly One-on-One Support
One virtual coaching session each month with your Clinical Exercise Physiologist and/or Certified Health Coach.
Review progress, discuss challenges, update goals, and make program adjustments as needed.
Exercise Progression
Your exercise prescription is reviewed and updated periodically to ensure continued progression, safety, and variety based on your goals, functional abilities, and medical status.
Continued Wellness Support
Receive ongoing guidance in:
Exercise progression
Nutrition
Strength training
Walking and physical activity
Stress management
Sleep and recovery
Healthy aging
Chronic disease self-management

Monthly habit review
