This program is ideal for those managing obesity, diabetes, hypertension, cardiovascular disease, cancer recovery, frailty, or other chronic conditions, as well as anyone beginning a healthier lifestyle who would benefit from expert guidance and accountability.
Whether you have been referred by your healthcare provider or are seeking support on your own, Phase 1: Begin is designed to complement your current medical care—not replace it. Research consistently demonstrates that regular physical activity and healthy lifestyle behaviors can improve function, quality of life, and outcomes across many chronic conditions.

What You'll Get
Duration: 4 weeks (fully virtual)
Weekly Virtual Support
Weekly one-on-one check-ins with your Clinical Exercise Physiologist and/or Certified Health Coach to review progress, answer questions, overcome barriers, and adjust your plan.
One instructor-led virtual exercise session each week designed for safety, proper technique, and gradual progression.
Health Education
Evidence-based education delivered throughout the program covering:
Understanding your medical condition and the role of lifestyle medicine
Exercise fundamentals and safe physical activity
Nutrition basics
Stress management
Sleep and recovery
Building healthy habits
Goal setting and behavior change strategies
Habit Tracking
Simple tracking tools to monitor:
Daily movement
Exercise participation
Nutrition habits
Sleep
Stress
Symptoms (when appropriate)
Your progress is reviewed weekly to identify trends, celebrate successes, and make individualized adjustments.
Exercise Prescription
An individualized exercise plan developed by your Clinical Exercise Physiologist, including:
Walking or aerobic activity prescription
Introductory strength training
Functional movement exercises
Mobility and flexibility
Modifications based on your medical history, current treatment, and physical abilities
Personalized Coaching
Receive guidance from both your Clinical Exercise Physiologist and Certified Health Coach to help you:
Set realistic goals
Build confidence
Develop healthy routines
Overcome barriers
Stay motivated throughout the program

Your Care Team
Clinical Exercise Physiologist (CEP)
Provides individualized exercise prescription, clinical oversight, functional assessment, exercise progression, and condition-specific modifications based on your health status.
Certified Health Coach
Provides behavior change coaching, goal setting, accountability, motivation, habit development, and practical strategies to help you successfully incorporate healthy lifestyle behaviors into everyday life.
Together, your care team combines clinical expertise with ongoing support to help you safely begin your journey toward better health.

Enrollment is available by self-referral or referral from your healthcare provider.
Every participant begins with an initial assessment to review medical history, health goals, current activity level, and readiness for change, allowing us to develop a personalized wellness plan that meets your individual needs.
